Youth Partner

FT, PT and On-Call. Responsible for the supervision of youth (males, ages 12-18) in a residential group home. Behavioral Health Specialist (Bilingual Preferred)

FT/40 hrs. Provides direct service to children and families as identified in an individualized Wraparound plan of care. This position works independently in the family’s home and in a variety of community and/or treatment settings. Provides support services and skills training to children and family members, including but not limited to, behavioral modification programs, independent living skills, home management/household services, transportation assistance, and temporary in-home caretaker. Qualified candidates must have Bachelor’s degree in Social Sciences and 1 year of experience working with emotionally disturbed children in an institutional setting, OR 4 years of experience working with emotionally disturbed children in an institutional setting. Facilitator/Case Manager

FT/40 hrs. Facilitate Wraparound process for Child and Family Teams. Maintain highly coordinated communication with all family members, system partners, and other individuals involved in the life of the child and family. Guide the Child and Family Teams in the development of Plans of Care and Crisis and Safety Plans according to Wraparound timelines. Qualified applicants must have a Bachelor’s degree with 2 years of social work or casework employment in the field of family or child welfare services OR a Master’s degree in a related field. Foster and Adoptions

ISFC Nurse

On-Call/5-10 hrs. Responsible for monitoring medical issues in foster homes including but not limited to the oversight of Individualized Health Care Plan, medication documentation, and medical resource development. On-call consultation with foster parents and Case Managers, not including emergency services. Provide additional medical training to foster parents and staff, or identify resources when specific training is required. Liaison between community medical service providers, psychiatrist, county agencies, client, pharmacy, and Olive Crest. Develop community medical resources for clients to ensure quality care (medical and dental). Provide staff and clients with training on medically related issues (i.e. universal precautions, AIDS/HIV information, etc.). Present the necessary information at the initial training of new employees (i.e., medication policy and documentation, AIDS/HIV information, etc.) Qualified applicants must be Licensed Vocational Nurse or Registered Nurse with two years of pediatric experience.
